Prednisolone Dosing Guidelines for Children
Initial Treatment of Nephrotic Syndrome (KDIGO)
- The KDIGO guideline recommends oral prednisolone 60 mg/m² per day or 2 mg/kg/day (maximum 60 mg/day) as a single daily dose for the initial episode of nephrotic syndrome. 1
- The recommended regimen is 4–6 weeks of daily dosing, followed by alternate‑day dosing at 40 mg/m² or 1.5 mg/kg (maximum 40 mg on alternate days) for 2–5 months with tapering. 1
- A minimum total treatment duration of 12 weeks is advised to lower relapse rates. 1
Relapse Management (KDIGO)
- For infrequent relapses, give the same 2 mg/kg/day until remission (≥ 3 days), then switch to alternate‑day dosing at 1.5 mg/kg for at least 4 weeks. 5
- For frequent relapses or steroid‑dependent disease, continue daily prednisolone until remission (≥ 3 days) and then maintain alternate‑day therapy for at least 3 months. 5
Acute Asthma Exacerbations (National Heart, Lung, and Blood Institute)
- The NHLBI recommends 1–2 mg/kg/day (maximum 60 mg/day) for 3–10 days for acute asthma exacerbations. 7
Practical Dosing Calculations for a 13.9 kg Child (Kidney International)
| Indication | Dose (mg/kg) | Total mg/day | Approx. volume of 15 mg/5 mL suspension (3 mg/mL) |
|---|---|---|---|
| Initial/relapse daily therapy | 2 mg/kg | 27.8 mg | 9.3 mL once daily |
| Moderate asthma exacerbation | 1 mg/kg | 13.9 mg | 4.6 mL once daily |
| Alternate‑day dosing (relapse) | 1.5 mg/kg | 20.8 mg | 6.9 mL on alternate days |
- Calculations are based on the concentration of the suspension (3 mg/mL). 3
General Dosing Principles
- Single‑daily dosing is preferred over divided doses because it is equally effective and improves adherence. 3
- For most pediatric conditions, the maximum daily prednisolone dose is capped at 60 mg regardless of weight‑based calculations. 1
- Weight‑based dosing (mg/kg) is acceptable and simpler than body‑surface‑area dosing; both are endorsed by KDIGO. 5
- In significantly overweight children, use ideal body weight to avoid unnecessary steroid exposure. 5
- Tapering is not required for short courses (< 1 week), especially when the child is concurrently on inhaled corticosteroids. 7
