Tamoxifen and Palbociclib Combination Therapy

Guideline Recommendations

  • The National Comprehensive Cancer Network (NCCN) guidelines support palbociclib with endocrine therapy for premenopausal women when combined with ovarian suppression, with palbociclib plus fulvestrant being a category 1 option for premenopausal women receiving ovarian suppression with an LHRH agonist 1, 2
  • The American Society of Clinical Oncology (ASCO) guidelines recommend that palbociclib may be offered in combination with fulvestrant in patients exposed to prior hormone therapy, applicable to premenopausal women undergoing ovarian suppression 3, 4

Dosage and Administration

  • Palbociclib should be administered 125 mg orally daily for 21 days every 28 days 3
  • Monitor complete blood counts on day 14 of the first two cycles, then at the start of each subsequent 28-day cycle 3, 4

Treatment Duration and Discontinuation

  • Treatment should continue until unequivocal disease progression documented by imaging, clinical examination, or disease-related symptoms 4
  • If disease progresses on palbociclib plus one endocrine agent, there are no data to support switching to another palbociclib-containing regimen 5
