Antibiotic Cross-Reactivity and Safety of Nitrofurantoin

Understanding Cross-Reactivity Patterns

  • Cross-reactivity between different beta-lactam antibiotics is primarily related to similar chemical structures, particularly the side chains, according to the Clinical Microbiology and Infection guidelines 1, 2
  • Patients with penicillin allergy can safely receive monobactams as there is minimal cross-reactivity, as stated by the Clinical Microbiology and Infection guidelines 2, 3

Nitrofurantoin Safety in Penicillin-Allergic Patients

  • There is no evidence of cross-reactivity between nitrofurantoin and penicillins in any of the clinical guidelines, including those from the Clinical Microbiology and Infection 1, 4, 5, 6

Clinical Approach to Antibiotic Selection

  • For patients requiring beta-lactam antibiotics despite penicillin allergy, skin testing and/or graded challenges may be appropriate depending on the severity and timing of the previous reaction, as recommended by the MMWR Recommendations and Reports 7
