Optimal Timing and Perioperative Management of Rivaroxaban

Dosing Timing and Food Requirements

  • The American College of Cardiology recommends that rivaroxaban be taken once daily with the evening meal to achieve adequate drug absorption. 1, 2, 3
  • Rivaroxaban must be administered with food; taking it without a concurrent meal leads to insufficient absorption. 1, 3
  • The guideline wording was updated from “large meal” to the more specific “evening meal” to provide clearer dosing instructions. 1, 2

Renal‑Adjusted Dosing

  • The evening‑meal timing recommendation applies to both the standard dose (20 mg once daily) and the reduced dose (15 mg once daily) that is used for patients with moderate renal impairment (creatinine clearance 15–50 mL/min). 1, 2

Peri‑operative Resumption

  • Low‑bleeding‑risk surgeries – rivaroxaban may be restarted at the full dose (20 mg once daily) the day after surgery (approximately 24 hours post‑operatively). 4, 5, 6, 7
  • High‑bleeding‑risk surgeries – rivaroxaban should be resumed 2–3 days after surgery (48–72 hours post‑operatively) at the full dose (20 mg once daily); alternatively, a reduced dose (10 mg once daily) may be used for the first two days in patients at high risk of thromboembolism. 4, 5, 6, 7

Avoiding Inadequate Absorption

  • Patients should never take rivaroxaban without food, as omission of a meal markedly reduces drug absorption. 1, 3
