Rasburicase Indications and Usage
Introduction to Rasburicase
- The European Society for Medical Oncology recommends rasburicase for the treatment of tumor lysis syndrome (TLS) in patients at high risk of developing TLS, as well as for adults with laboratory TLS and children with high risk of TLS or rapid worsening of TLS biochemical parameters 1, 2
Indications for Rasburicase
- The American Society of Clinical Oncology recommends rasburicase for the treatment of clinical TLS, along with adequate hydration, in all patients presenting with clinical TLS 1, 2
- Rasburicase is indicated for adults with laboratory TLS, characterized by the presence of at least two biochemical abnormalities, including hyperkalemia, hyperphosphatemia, hypocalcemia, and hyperuricemia 1, 3
- The National Comprehensive Cancer Network recommends rasburicase for prophylaxis in pediatric patients at high risk of developing TLS and to prevent acute hyperuricemia 3, 4
Comparison with Allopurinol
- Rasburicase acts rapidly, degrading uric acid to allantoin, allowing for quicker continuation of chemotherapy, and is significantly more effective than allopurinol in reducing uric acid levels in patients with hyperuricemia due to TLS 4, 6, 7
- Rasburicase decreases uric acid levels by 86% within 4 hours after the first dose, compared to only 12% in patients treated with allopurinol 7
Dosage and Administration
- The standard dose of rasburicase is 0.20 mg/kg/day, administered intravenously over 30 minutes, with the first dose given at least 4 hours before the start of specific anti-tumor therapy, and continued for at least 3-5 days 3
Contraindications and Precautions
- Patients with a history of previous reactions to rasburicase, such as anaphylaxis, severe hypersensitivity, hemolysis, or methemoglobinemia, should not receive rasburicase 3
- The European Medicines Agency recommends that patients with methemoglobinemia or metabolic disorders that may cause hemolytic anemia should not receive rasburicase 3
Special Considerations
- The duration of rasburicase treatment should be based on control of plasma uric acid levels, and treatment is not necessary when uric acid is extremely low or no longer detectable 5
- The use of allopurinol after initial treatment with rasburicase is not necessary, according to expert panels, although there are no data to demonstrate whether patient outcomes are better or worse with allopurinol after initial treatment with rasburicase 5
