Best Oral Antibiotics for UTI in Patients with Elevated Liver Enzymes

First-Line Options

  • The European Association of Urology recommends fosfomycin trometamol as the safest and most effective option for patients with hepatic impairment, with a 3g single dose oral sachet being the preferred treatment 1
  • Nitrofurantoin is an alternative first-line option, with a recommended dose of 100mg twice daily for 5 days, and is contraindicated in severe hepatic impairment or if creatinine clearance <30 mL/min, as stated by the European Urology guidelines 1, 2, 3

Second-Line Options

  • Pivmecillinam may be considered as a second-line option, with limited hepatic metabolism, and requires monitoring of liver function during treatment, according to the European Urology guidelines 1
  • Aminoglycosides, such as gentamicin, may be considered for uncomplicated cystitis, with microbiological cure rates of 87-100% reported, and are primarily renally excreted with minimal hepatic metabolism, as stated by the Journal of Microbiology, Immunology and Infection 4, 5

Antibiotics to Avoid

  • Fluoroquinolones are not recommended due to the risk of hepatotoxicity and FDA warnings, with significant adverse effects and collateral damage, as stated by The Journal of Urology 6

Treatment Algorithm

  • The European Urology guidelines recommend confirming diagnosis with urine culture before starting antibiotics if possible, and assessing severity to determine the appropriate treatment duration 1, 3
  • The treatment duration for uncomplicated cystitis is a single dose of fosfomycin or 5-day nitrofurantoin, while complicated UTI requires extended therapy based on clinical response, as stated by the JAMA network open 1, 7

Special Considerations

  • For recurrent UTIs, prophylactic strategies that minimize hepatic impact, such as methenamine hippurate, may be considered, according to the European Urology guidelines 1
