Amlodipine for Hypertension Treatment

Clinical Efficacy

  • The American Society of Nephrology recommends amlodipine as an effective treatment for hypertension, with dose-dependent effects starting at 2.5 mg and optimal effects typically seen at 5-10 mg daily, as demonstrated in the ALLHAT trial where amlodipine was equally effective as chlorthalidone in preventing coronary heart disease mortality and morbidity 1
  • The American Heart Association suggests that amlodipine-based therapy demonstrates superiority over atenolol-based therapy in reducing stroke and mortality, as shown in the ASCOT trial 2
  • The American Stroke Association notes that amlodipine-based therapy reduces the incidence of fatal and nonfatal strokes by 25% compared to atenolol-based therapy in diabetic patients, as demonstrated in the Anglo-Scandinavian Cardiac Outcomes Trial 3

Advantages in Special Populations

  • The European Society of Cardiology recommends amlodipine as a step 3 agent in heart failure patients with persistent hypertension despite treatment with ACE inhibitors, beta-blockers, and mineralocorticoid receptor antagonists 4, 5

Combination Therapy

  • The American Society of Nephrology highlights the importance of combination therapy in managing hypertension, as demonstrated in the ALLHAT study where many patients required multiple drugs to achieve blood pressure control 1

Clinical Considerations and Caveats

  • The European Society of Cardiology advises against using negatively inotropic CCBs (diltiazem and verapamil) to treat hypertension in patients with heart failure with reduced ejection fraction, instead recommending amlodipine as a suitable alternative 4, 5

Outcomes Benefits

  • The European Society of Cardiology notes that lowering blood pressure with amlodipine reduces the risk of fatal and nonfatal cardiovascular events, primarily strokes and myocardial infarctions, as demonstrated in the VALUE trial where amlodipine was compared to valsartan and showed similar efficacy in preventing cardiac events and death 6
