Fluid Management in Heart Failure Patients with AKI

Assessment of Volume Status

  • Careful evaluation of volume status is essential before administering fluids to heart failure patients with AKI, as recommended by the American Journal of Kidney Diseases and Kidney International 1, 2

Fluid Management Principles

  • Fluid administration may be appropriate in heart failure patients with AKI who show signs of hypotension unresponsive to vasopressors, as suggested by Kidney International 3
  • When fluid administration is necessary, isotonic crystalloids should be the first choice, as recommended by the American Journal of Kidney Diseases and Kidney International 1, 2
  • Avoid hydroxyethyl starch solutions as they are associated with increased risk of AKI and mortality, according to Kidney International 2
  • Avoid albumin in patients with heart failure, as it shows no benefit over crystalloids and increases costs, as stated by the British Journal of Anaesthesia 4
  • Avoid potassium-containing balanced solutions in patients with hyperkalemia risk, as advised by Kidney International 5

Management of Fluid Overload

  • Diuretics should not be used to prevent AKI but are appropriate for managing volume overload, as recommended by the American Journal of Kidney Diseases 1
  • Protocol-based management of hemodynamic parameters may help prevent worsening of AKI, according to the American Journal of Kidney Diseases 1

Special Considerations

  • In patients with AKI and heart failure, vasopressors in conjunction with minimal fluids may be more appropriate than large-volume fluid resuscitation, as suggested by the American Journal of Kidney Diseases and Kidney International 1, 3
