Curcumin and Apixaban Interaction

Mechanism of Potential Interaction

  • Apixaban is a substrate of both CYP3A4 and P-glycoprotein (P-gp), making it vulnerable to interactions with inhibitors of these pathways, such as curcumin 1, 2

Practical Management Recommendations

  • Monitor closely for signs of bleeding, including unusual bruising, prolonged bleeding from cuts, blood in urine or stool, or unexplained bleeding, when combining curcumin with apixaban 3
  • Seek emergency care for unusual bleeding or bruising, particularly if severe or unexplained, when taking apixaban with curcumin 4
  • Watch for signs of major bleeding including hemodynamic instability, bleeding at critical sites, or hemoglobin decrease ≥2 g/dL, when combining apixaban with curcumin 3
  • Report any changes in bleeding patterns to healthcare providers before surgical or dental procedures when taking apixaban with curcumin 4

Alternative Considerations

  • Acetaminophen is considered safe with apixaban and does not increase bleeding risk, and can be used as an alternative for anti-inflammatory effects 4
  • Edoxaban may be a safer DOAC alternative for patients requiring both anticoagulation and supplements with potential interactions, as it demonstrates fewer clinically significant drug interactions 5, 6
