Enoxaparin Initiation in Patients Already on Prasugrel and Plavix

Critical Bleeding Risk Assessment

  • For patients weighing less than 60 kg on dual antiplatelet therapy, the American College of Cardiology recommends reducing enoxaparin to 30 mg subcutaneously once daily due to the combined bleeding risk from low body weight, antiplatelet therapy, and potential renal impairment 1
  • The National Comprehensive Cancer Network recommends that patients weighing less than 50 kg have increased bleeding risk even with standard prophylactic dosing, and therefore require dose adjustment 2, 1
  • The American Heart Association recommends reducing enoxaparin to 30 mg subcutaneously once daily for patients weighing 50-60 kg on dual antiplatelet therapy, as this dose provides adequate prophylaxis while minimizing hemorrhagic complications 1
  • For patients with creatinine clearance less than 30 mL/min, the Mayo Clinic recommends using 30 mg subcutaneously once daily, which is both the weight-based and renal-adjusted dose 3, 1, 4

Monitoring Requirements

  • The American College of Chest Physicians recommends monitoring platelet counts every 2-3 days from day 4 to day 14 to screen for heparin-induced thrombocytopenia (HIT), as the risk may be elevated with concurrent antiplatelet therapy 3, 1
  • The National Comprehensive Cancer Network recommends considering monitoring anti-Xa levels if creatinine clearance is less than 30 mL/min, or if clinical concerns about efficacy or bleeding arise 1, 4, 5

Critical Safety Considerations

  • The American Heart Association recommends not initiating enoxaparin if active bleeding is present, platelet count is less than 50,000/μL, or recent intracranial hemorrhage or high bleeding risk procedure has occurred 1, 3
  • The American College of Cardiology recommends evaluating whether both antiplatelet agents are absolutely necessary, and considering discontinuing prasugrel while maintaining Plavix during hospitalization 1

Alternative Strategies

  • The National Comprehensive Cancer Network recommends considering mechanical prophylaxis instead of pharmacologic prophylaxis if the patient has very high bleeding risk, creatinine clearance is less than 15 mL/min, or history of HIT 5, 1, 4, 2
  • The American College of Chest Physicians recommends using unfractionated heparin 5000 units subcutaneously every 8-12 hours as an alternative to enoxaparin if creatinine clearance is less than 30 mL/min or significant bleeding concerns exist 2, 4, 3
