Systemic Light Chain Amyloidosis Management
Diagnostic Confirmation
- The International Myeloma Society recommends tissue biopsy with Congo red staining showing apple-green birefringence under polarized light for diagnostic confirmation 2
- Mass spectrometry (LC-MS/MS) is the gold standard for amyloid typing with 88% sensitivity and 96% specificity, and is recommended by the National Comprehensive Cancer Network 1, 2
- Complete monoclonal protein screening, including serum free light chain assay, serum immunofixation electrophoresis, and urine immunofixation electrophoresis, is necessary for diagnosis, as stated by the American Society of Clinical Oncology 1, 2
- The National Comprehensive Cancer Network advises against relying on standard protein electrophoresis alone for screening, as it misses monoclonal spikes in nearly 50% of AL amyloidosis cases 3, 4, 2
Initial Treatment
- The American College of Cardiology recommends Daratumumab-CyBorD as the preferred first-line treatment for both ASCT-eligible and ASCT-ineligible patients, achieving very good partial response or better in 78.5% of patients 1, 2
- The European Society for Medical Oncology suggests that high-dose melphalan followed by ASCT should be considered for highly selected patients who meet strict eligibility criteria, including age <60 years, ≤2 organs involved, and good performance status 1, 2, 7, 5
Organ Involvement Assessment
- The International Myeloma Society recommends cardiac assessment, including NT-proBNP and troponin T levels, echocardiography, and cardiac MRI, to determine the extent of organ damage 5, 6, 1
- The National Kidney Foundation advises renal assessment, including 24-hour urine protein collection, serum creatinine, and BUN, to evaluate renal function 3, 4
Treatment Response Monitoring
- The American Society of Clinical Oncology recommends response assessment at 3-6 months using the difference between involved and uninvolved free light chains (dFLC) 2, 1
- The European Society for Medical Oncology defines complete response, very good partial response, partial response, and no response based on dFLC levels and organ-specific response 2, 1
Prognosis and Treatment Goals
- The American College of Cardiology states that cardiac involvement is the main driver of disease prognosis and mortality, with approximately 30% of patients dying within the first year from diagnosis due to advanced cardiac involvement 5, 1
- The International Myeloma Society recommends that the primary goal of treatment is to eradicate pathological plasma cells and remove amyloidogenic light chains from circulation, with deep hematologic responses directly correlating with improved organ function and survival 1, 2
