Amoxicillin Dosing for Adult Otitis Media

First-Line Treatment Options

  • The American Academy of Otolaryngology-Head and Neck Surgery recommends amoxicillin as the first-line treatment for acute otitis media in adults due to its effectiveness against common pathogens, safety profile, low cost, and narrow microbiologic spectrum 3, 4
  • For standard-risk adults with no recent antibiotic use, the recommended dose of amoxicillin is 1.5-4 g/day divided into 2-3 doses 1, 2
  • For high-risk patients, high-dose amoxicillin at 4 g/day is recommended 1, 4

Alternative Treatment Options for Penicillin Allergy

  • For patients with non-type I penicillin allergy, the American Academy of Otolaryngology-Head and Neck Surgery recommends cefdinir (300 mg twice daily or 600 mg once daily) as an appropriate alternative 1, 5
  • Other alternatives include cefuroxime axetil (500 mg twice daily) or cefpodoxime proxetil 1, 4

Treatment Duration and Monitoring

  • The American Academy of Family Physicians recommends a standard treatment duration of 5-10 days 3, 4
  • Patients should be reassessed if there is no clinical improvement after 48-72 hours of initial therapy 1, 4
  • If treatment failure occurs after 72 hours, switching to an alternative antibiotic is recommended 3, 1

Second-Line Treatment Options

  • The World Health Organization recommends amoxicillin-clavulanate (4 g/250 mg per day) as the second-line therapy for treatment failures with first-line amoxicillin 2, 4
  • For patients who have received antibiotics in the previous 4-6 weeks, starting with amoxicillin-clavulanate may be more appropriate 1

Special Considerations

  • The maximum standard adult dose of amoxicillin is typically considered to be 1500 mg/day for standard dosing and 4000 mg/day for high-dose therapy 1

Watchful Waiting Approach

  • The American Academy of Family Physicians recommends a watchful waiting approach for otherwise healthy adults with mild symptoms to reduce unnecessary antibiotic use 2, 3
