Ethambutol Dosing in Renal Failure

Pharmacokinetic Considerations

  • Ethambutol is approximately 80% cleared by the kidneys and may accumulate in patients with renal insufficiency 2, 3

Dosing Recommendations Based on Renal Function

  • For patients with creatinine clearance <30 mL/min or on hemodialysis, the dosing frequency should be changed from daily to three times weekly, with a dose of 20-25 mg/kg/dose 1, 2, 3, 4
  • For patients with creatinine clearance <30 mL/min or on hemodialysis, medications should be administered after hemodialysis on dialysis days to avoid premature clearance 2, 3
  • For patients with creatinine clearance 30-50 mL/min, standard doses can be used, but measurement of serum concentrations 2 and 6 hours after administration is recommended to optimize dosing 2, 3
  • Close monitoring for toxicity is essential in patients with creatinine clearance 30-50 mL/min 1

Monitoring Recommendations

  • Serum ethambutol levels should be measured in patients with renal dysfunction 5
  • Serum drug concentrations should be monitored to ensure adequate absorption without excessive accumulation 2, 3
  • Visual acuity and color vision should be assessed before starting treatment, and regular ophthalmologic monitoring is crucial as ethambutol can cause optic neuritis and retrobulbar neuritis 5

Clinical Considerations and Pitfalls

  • Decreasing the dose (rather than extending the interval) lowers peak serum drug concentrations and can compromise treatment efficacy 2, 3
  • Simply reducing the dose is not recommended as it may lead to subtherapeutic levels and treatment failure 4
  • For patients on peritoneal dialysis, doses recommended for hemodialysis patients should be used, and adequacy should be verified using serum concentration monitoring 2
