Blood Glucose Management in Patients on Dexamethasone

General Glycemic Targets

  • The American Diabetes Association recommends target blood glucose levels of 80-180 mg/dL (4.4-10.0 mmol/L) for patients on dexamethasone, with a wider acceptable range of 70-200 mg/dL (3.9-11.1 mmol/L) depending on individual risk factors for hypoglycemia 1, 2
  • Target preprandial glucose levels should be 80-130 mg/dL (4.4-7.2 mmol/L) for patients on dexamethasone 1, 3
  • Target postprandial glucose levels should be <180 mg/dL (10.0 mmol/L) for patients on dexamethasone 1, 3
  • For patients with higher risk of hypoglycemia or severe comorbidities, higher glucose targets up to 200 mg/dL (11.1 mmol/L) may be acceptable, as recommended by The Lancet Diabetes and Endocrinology 4

Monitoring and Treatment Recommendations

  • More frequent monitoring is necessary for patients on dexamethasone due to its significant hyperglycemic effects, with monitoring every 2-4 hours initially 5, 2
  • The American Diabetes Association recommends treating hypoglycemia with 15-20g of glucose for conscious patients with glucose <70 mg/dL (3.9 mmol/L) 1, 6
  • Recheck glucose after 15 minutes and repeat treatment if hypoglycemia persists, as recommended by the American Diabetes Association 1

Special Considerations for Different Patient Populations

  • For non-critically ill hospitalized patients, target glucose range should be 80-180 mg/dL (4.4-10.0 mmol/L), as recommended by the American Diabetes Association 2
  • For critically ill patients, target glucose range should be 140-180 mg/dL (7.8-10.0 mmol/L), as recommended by The Lancet Diabetes and Endocrinology 4
  • Avoid glucose values >180 mg/dL (10.0 mmol/L) and <110 mg/dL (6.1 mmol/L) in critically ill patients, as recommended by The Lancet Diabetes and Endocrinology 4

Insulin Management

  • For patients on dexamethasone requiring insulin, consider a regimen that addresses the pattern of steroid-induced hyperglycemia, as recommended by the American Diabetes Association 2
  • NPH insulin may be particularly useful for managing daytime hyperglycemia from morning steroid doses, as recommended by the American Diabetes Association 2
  • For patients on continuous insulin infusion, target glucose range should be 140-180 mg/dL (7.8-10.0 mmol/L), as recommended by The Lancet Diabetes and Endocrinology 4

Hypoglycemia Risk Management

  • The American Diabetes Association classifies hypoglycemia as Level 1: Glucose <70 mg/dL (3.9 mmol/L) but ≥54 mg/dL (3.0 mmol/L), Level 2: Glucose <54 mg/dL (3.0 mmol/L), and Level 3: Severe event requiring assistance 1
  • Prescribe glucagon for patients at risk of severe hypoglycemia, as recommended by the American Diabetes Association 1
