Lidocaine Jelly Dosing and Safety for Urethral Catheterization

Male Catheterization – Administration Timing

  • After instilling 2 % lidocaine jelly into the male urethra, wait 5–10 minutes before inserting the catheter to achieve optimal mucosal anesthesia. 1

Pediatric Dosing – Weight‑Based Limits

  • For children, the maximum lidocaine dose is 4.5 mg kg⁻¹ of body weight for topical/mucosal application. 1
  • In children under 10 kg, use a 1–2 % lidocaine concentration (rather than 2 %) to allow an appropriate volume while staying within safe dose limits. 2

Maximum Dosing Limits – Adults and Children

  • Adults: do not exceed 4.5 mg kg⁻¹ without epinephrine or 7.0 mg kg⁻¹ with epinephrine for topical/mucosal lidocaine. [3][4]5
  • Children: limit to 1.5–2.0 mg kg⁻¹ without epinephrine or 3.0–4.5 mg kg⁻¹ with epinephrine for topical/mucosal lidocaine. [3][4]5

Toxicity Prevention – Practical Measures

  • Use the lowest effective dose of lidocaine jelly to minimize risk of systemic toxicity. 4
  • Monitor patients for early signs of local anesthetic systemic toxicity, including circumoral numbness, metallic taste, tinnitus, and confusion. 4
  • Avoid concurrent use of other lidocaine‑containing products (e.g., topical anesthetics, sprays) during the same procedure. [1][6]

Documentation – Dose Recording

  • Record the total lidocaine dose administered for each patient, especially when multiple procedures or different lidocaine formulations are used, to ensure cumulative exposure remains within safe limits. 2
