Escitalopram Dosing and Administration

Discontinuation and Tapering

  • The American Family Physician recommends tapering escitalopram gradually over 10-14 days minimum to limit withdrawal symptoms, and if intolerable symptoms occur during taper, resume the previous dose and decrease more gradually 1, 2

Special Considerations

  • The American Family Physician suggests that patients with severe renal impairment should use escitalopram with caution, and that tapering gradually rather than stopping abruptly can minimize discontinuation symptoms including irritability, agitation, dizziness, sensory disturbances, anxiety, confusion, headache, lethargy, emotional lability, insomnia, and hypomania 1

Escitalopram Dosing Considerations

Special Populations

  • Lower starting doses of escitalopram are often recommended for elderly patients, approximately 50% of the adult starting dose, due to greater risk of adverse drug reactions, and dose increases should be made cautiously in this population, as suggested by the American Family Physician 3
  • For patients with major depressive disorder, the American Family Physician recommends continuing treatment for 4-12 months after remission for the first episode, and longer maintenance treatment may be beneficial for recurrent depression 3

Escitalopram Dosing and Safety Considerations

Dosing Recommendations

  • The European Heart Journal recommends limiting the maximum dose of escitalopram due to QT-interval prolongation concerns, with a further reduction in patients older than 60 years 4
  • The American Academy of Clinical Psychiatrists recommends a therapeutic plasma concentration range of 15-80 ng/mL for escitalopram, based on steady-state pharmacokinetic studies at therapeutically effective doses, with a Level 4 recommendation 5

Safety Considerations

  • The European Society of Cardiology classifies escitalopram as having a QT-prolongation risk, though less pronounced than some other antidepressants, and recommends caution in patients with pre-existing QT interval prolongation 4
  • The National Institute for Health and Care Excellence recommends tapering escitalopram gradually over a minimum of 10-14 days to avoid withdrawal symptoms, with a recommendation to resume the previous dose and decrease more gradually if intolerable symptoms occur 4
