Cough Management in COPD Patients

Key Safety Considerations

  • Local anesthetics like benzonatate can theoretically worsen respiratory function in patients with severe hypoxia or hypercapnia, similar to concerns noted with other anesthetic agents in respiratory disease 1

Alternative Cough Management Approaches

  • The American Thoracic Society recommends optimizing bronchodilator therapy first before adding cough suppressants, as improved airflow may reduce cough, with long-acting muscarinic antagonists (LAMAs) preferred over long-acting beta-agonists (LABAs) for reducing exacerbations 2, 3
  • The European Respiratory Society suggests that LABA/LAMA combinations improve symptoms more effectively than monotherapy 3

Clinical Decision Algorithm

  • The European Respiratory Society recommends assessing current respiratory status and optimizing COPD maintenance therapy, ensuring patients are on appropriate long-acting bronchodilators (LAMA preferred, or LAMA/LABA combination) 4, 2, 3

Important Pitfalls to Avoid

  • The European Respiratory Society advises against combining benzonatate with benzodiazepines or opioids, and suggests that morphine and other opioids should only be used in terminal COPD for dyspnea suppression, as they carry high risk of respiratory depression 5, 4
  • Physiotherapy techniques (coughing, forced expiratory maneuvers, pursed-lip breathing) should be employed as adjunctive non-pharmacologic cough management, as recommended by the European Respiratory Society 5, 4
