Contraindications and Precautions for Amlodipine Use

Clinical Situations Where Amlodipine Should NOT Be Used

  • Amlodipine is NOT recommended as treatment for heart failure, though it is not contraindicated for blood pressure control in these patients, as stated by the American Heart Association 1
  • Non-dihydropyridine calcium channel blockers (diltiazem, verapamil) are absolutely contraindicated in Heart Failure with Reduced Ejection Fraction (HFrEF) due to negative inotropic effects and increased mortality risk, according to the American College of Cardiology 1
  • Amlodipine can be safely used in heart failure patients when needed for hypertension or angina, as demonstrated in the PRAISE trials, with a moderate strength of evidence 1, 2

Pediatric and Special Population Considerations

  • Amlodipine is contraindicated in children under 1 year of age, as recommended by the American Academy of Pediatrics 3
  • For children 6-17 years, dosing should start at 2.5 mg daily with a maximum of 5 mg daily, according to the American Academy of Pediatrics 3

Relative Contraindications and Cautions

  • Amlodipine is contraindicated in advanced aortic stenosis, due to the risk of dangerous hypotension, as warned by the American Heart Association 4
  • The vasodilatory effects of amlodipine can cause hypotension in patients who depend on preload to maintain cardiac output, particularly in those with advanced aortic stenosis, with a high strength of evidence 4

Key Differentiating Points from Other Calcium Channel Blockers

  • Unlike non-dihydropyridines (verapamil, diltiazem), amlodipine does NOT cause bradycardia, heart block, or worsen heart failure through negative inotropic effects, as stated by the American College of Cardiology 1
  • Amlodipine's contraindication profile is much narrower than other calcium channel blockers, with a low risk of cardiac conduction disturbances, according to the American Heart Association 1

Special Considerations for Other Conditions

  • Vasospastic angina is a contraindication for beta-blockers, not for amlodipine, and calcium channel blockers like amlodipine are actually preferred in this condition, with a high strength of evidence 5

Amlodipine Safety in Patients with AV Block

Mechanism and Effects

  • Dihydropyridines, such as amlodipine, are highly selective for arterial/arteriolar smooth muscle and produce marked peripheral vasodilation with no significant AV nodal or sinoatrial nodal effects, according to the American Heart Association 6
  • Non-dihydropyridines, such as verapamil and diltiazem, have prominent negative chronotropic and dromotropic effects on AV nodal conducting tissue and may predispose to high-degree AV block when administered to patients with preexisting AV nodal disease, as stated by the American College of Cardiology 6

Guideline Recommendations

  • The American College of Cardiology recommends avoiding non-dihydropyridines in patients with clinically significant LV dysfunction, increased risk for cardiogenic shock, PR interval greater than 0.24 seconds, or second- or third-degree AV block without a cardiac pacemaker 7
  • The American Heart Association suggests that amlodipine is not contraindicated in patients with AV block, unlike non-dihydropyridine calcium channel blockers, due to its minimal effects on cardiac conduction tissue 8, 7

Clinical Considerations

  • When selecting calcium channel blockers in patients with or at risk for AV block, the European Society of Cardiology recommends choosing amlodipine (or other dihydropyridines) for hypertension or angina management, and avoiding verapamil or diltiazem if PR interval >0.24 seconds or any degree of AV block exists 6, 7
  • The American College of Cardiology advises being cautious with CYP3A inhibitors (such as clarithromycin, itraconazole, ritonavir) that may increase amlodipine levels, as this can lead to elevated amlodipine concentrations and potential adverse effects 9, 10
