Treatment for Subacute Combined Degeneration of the Cord

Diagnosis and Etiology

  • Demyelination of the dorsal and lateral spinal cord is a characteristic of subacute combined degeneration (SCD), a neurological complication of vitamin B12 deficiency 3

Treatment Protocol

  • The American College of Radiology recommends administering hydroxocobalamin 1 mg intramuscularly on alternate days until there is no further improvement for patients with neurological involvement 2
  • The treatment protocol involves continuing with hydroxocobalamin 1 mg intramuscularly every 2 months for maintenance for patients with neurological involvement 2
  • For patients without neurological involvement, the recommended treatment is hydroxocobalamin 1 mg intramuscularly three times a week for 2 weeks, followed by maintenance treatment of 1 mg intramuscularly every 2-3 months lifelong 2

Important Considerations

  • The American College of Radiology advises against giving folic acid first, as it may mask underlying vitamin B12 deficiency and precipitate subacute combined degeneration of the spinal cord 2

Common Pitfalls to Avoid

  • The Journal of the American College of Radiology notes that misdiagnosis can occur, as SCD can mimic other myelopathies, and MRI is useful for evaluation of the spinal cord 3
