Anti-Histone Antibody Testing in Suspected Lupus

Primary Indications for Testing

  • Anti-histone antibodies should primarily be ordered when drug-induced lupus is suspected, particularly in patients with symptoms of lupus who are taking medications known to cause drug-induced lupus erythematosus (DILE) 1, 2
  • Anti-histone antibodies can be used in patients with confirmed lupus nephritis who remain anti-dsDNA negative, as they are more prevalent in patients with lupus nephritis than in those without kidney disease 1, 2

Standard Lupus Antibody Panel vs. Anti-Histone Antibodies

  • The European League Against Rheumatism (EULAR) recommends a standard antibody panel for lupus diagnosis and monitoring, which includes ANA, anti-dsDNA, anti-Ro, anti-La, anti-RNP, anti-Sm, anti-phospholipid, C3, and C4 at baseline 3, 4
  • Anti-histone antibodies are NOT part of this standard panel, according to EULAR recommendations 3, 4

Lupus Nephritis Monitoring

  • Consider testing for anti-histone antibodies in patients with confirmed lupus nephritis who remain anti-dsDNA negative 1, 2
  • Anti-histone antibodies may help monitor disease activity when standard markers are not informative 1
  • Anti-histone antibodies should only be used when lupus nephritis is confirmed to be not secondary to drug treatment 1, 2
  • Positive anti-histone antibodies in lupus nephritis may indicate higher disease activity in the kidneys 1, 2

Diagnostic Approach to Suspected Drug-Induced Lupus

Initial Testing and Diagnostic Algorithm

  • The European League Against Rheumatism (EULAR) recommends beginning with ANA testing as the first-level test for suspected autoimmune disease, and if ANA is positive, proceeding with specific antibody testing, including anti-histone antibodies when drug-induced lupus is specifically suspected 5, 6, 7

Interpretation of Results and Clinical Considerations

  • The presence of anti-histone antibodies with negative or low anti-dsDNA titers strongly suggests drug-induced lupus, according to the Autoimmunity Reviews 7
  • Anti-histone antibody titers gradually fall after drug discontinuation, as shown in longitudinal studies 8
