Treatment of Uncomplicated Urinary Tract Infections

Diagnosis and Treatment

  • The European Association of Urology recommends first-line treatment for uncomplicated UTIs in women includes nitrofurantoin 100 mg twice daily for 5 days, fosfomycin trometamol 3 g single dose, or pivmecillinam 400 mg three times daily for 3-5 days 1, 2
  • Uncomplicated cystitis is defined as acute, sporadic, or recurrent cystitis in non-pregnant women without relevant anatomic/functional urinary tract abnormalities or comorbidities 1
  • Diagnosis can be made with high probability based on lower urinary tract symptoms (dysuria, frequency, urgency) and absence of vaginal discharge 1
  • The European Association of Urology recommends urine culture is recommended only in specific situations, such as suspected acute pyelonephritis, symptoms that don't resolve or recur within 4 weeks after treatment, women with atypical symptoms, or pregnant women 1

First-Line Treatment Options

  • Nitrofurantoin options include macrocrystals 50-100 mg four times daily for 5 days, monohydrate/macrocrystals 100 mg twice daily for 5 days, or macrocrystals prolonged release 100 mg twice daily for 5 days 1
  • Fosfomycin trometamol is recommended as a 3 g single dose 1
  • Pivmecillinam is recommended as 400 mg three times daily for 3-5 days 1

Alternative Treatment Options

  • Cephalosporins, such as cefadroxil, are recommended as 500 mg twice daily for 3 days if local E. coli resistance is less than 20% 1
  • Trimethoprim is recommended as 200 mg twice daily for 5 days, but not in the first trimester of pregnancy 1
  • Trimethoprim-sulfamethoxazole is recommended as 160/800 mg twice daily for 3 days, but not in the last trimester of pregnancy and only in regions where local E. coli resistance rates are below 20% 1, 2

Special Considerations

  • For men with UTIs, longer treatment duration is typically recommended 2
  • Trimethoprim-sulfamethoxazole is recommended as 160/800 mg twice daily for 7 days for men with UTIs 1
  • For treatment failures, urine culture and susceptibility testing are recommended, and retreatment with a 7-day regimen using another agent is suggested 1

Antimicrobial Resistance Considerations

  • Fluoroquinolones should be reserved for more invasive infections due to concerns about "collateral damage" and increasing resistance 2
  • Amoxicillin or ampicillin should not be used for empirical treatment due to poor efficacy and high prevalence of antimicrobial resistance 2

Follow-up Recommendations

  • Routine post-treatment urinalysis or urine cultures are not indicated for asymptomatic patients 1
  • Follow-up cultures are recommended only if symptoms persist or recur within 2-4 weeks after treatment 2
