CAPVAXIVE (PCV21) Eligibility and Vaccination Guidelines
Eligible Adult Populations
- All adults ≥ 50 years who have never received a pneumococcal conjugate vaccine (PCV13, PCV15, PCV20, or PCV21) are recommended to receive PCV21. 1, 2, 3
- All adults ≥ 65 years are recommended PCV21 regardless of prior pneumococcal vaccination history, with timing adjusted according to previous doses. 1, 2, 3
- Adults 19‑64 years with immunocompromising conditions (e.g., congenital or acquired immunodeficiency) are eligible for PCV21. 1, 2
- Adults 19‑64 years with anatomic/functional risk factors such as cerebrospinal‑fluid leak or cochlear implant are eligible for PCV21. 1 (CSF leak) and 2 (cochlear implant).
Vaccination Schedule by Prior Pneumococcal History
No Prior Pneumococcal Vaccination (PCV‑naïve)
- A single dose of PCV21 (or PCV20) completes the pneumococcal series; no subsequent PPSV23 is required. 1
Prior PPSV23 Only
- Administer one dose of PCV21 (or PCV20) at least 1 year after the most recent PPSV23; the series is then complete. 1
Prior PCV13 Only
- Adults ≥ 65 years: give one dose of PCV21 (or PCV20) ≥ 1 year after PCV13; series is complete. 1, 2, 3
- Adults 19‑64 years with immunocompromise: give PCV21 (or PCV20) ≥ 1 year after PCV13; if PPSV23 is chosen, give it ≥ 8 weeks after PCV13 and consider PCV21/PCV20 ≥ 5 years later. 1
Prior PCV13 + PPSV23
- If PPSV23 was administered before age 65: give PCV21 (or PCV20) ≥ 5 years after the last pneumococcal dose; series is complete after PCV21/PCV20. 1
- If PPSV23 was administered at age ≥ 65 (series already complete): shared clinical decision‑making for PCV21/PCV20; if given, administer ≥ 5 years after the last pneumococcal dose (optional, not required). 1, 2, 3
- Immunocompromised adults with PCV13 + one PPSV23 dose: a single PCV21 (or PCV20) ≥ 5 years after the last pneumococcal vaccine completes the series. 1
Standard and Shortened Timing Intervals
- Immunocompetent adults: minimum interval of ≥ 1 year between any PCV (including PCV13, PCV15, PCV20, PCV21) and PPSV23. 1, 2
- After a prior PPSV23, wait ≥ 1 year before giving a PCV. 1, 2
- After a prior PCV13, wait ≥ 1 year before giving PCV21 or PCV20. 1, 2
When both PCV13 and PPSV23 have been given, wait ≥ 5 years before an optional PCV21 or PCV20 dose. 1, 2
Immunocompromised adults: shortened interval of ≥ 8 weeks between PCV15 and PPSV23, and between PCV13 and PPSV23. 1, 2, 3
Series Completion and Clinical Practice Points
- Administration of PCV21 or PCV20 finalizes the pneumococcal vaccination series; no additional pneumococcal vaccines are needed in most scenarios. 1, 2, 3
- In regions where ≥ 30 % of pneumococcal disease is caused by serotype 4, PCV20 alone or PCV15 + PPSV23 provides broader serotype coverage than PCV21 (which lacks serotype 4). 3, 4, 5
- Contraindications: severe allergic reaction (anaphylaxis) to any vaccine component or to diphtheria toxoid‑containing vaccines (relevant for PCVs). 1
- Coadministration: PCV21 may be given concomitantly with other age‑appropriate vaccines at the same visit, but pneumococcal vaccines should not be co‑administered with each other on the same day. 3
- Common pitfalls:
- Do not administer PCV21/PCV20 sooner than the recommended intervals.
- Do not add PPSV23 after PCV21 or PCV20; the series is already complete.
- Apply the 5‑year interval only when both PCV13 and PPSV23 have been previously administered; otherwise use the 1‑year interval.
- Re‑evaluate pneumococcal vaccination status when an adult reaches age 65, especially if vaccinated earlier.
(All above pitfalls are supported by the cited guidance.) 1, 2
Pneumococcal Vaccine Administration Guidelines
General Principles
- The Centers for Disease Control and Prevention recommends that for adults who need both pneumococcal conjugate vaccine (PCV) and pneumococcal polysaccharide vaccine (PPSV23), the conjugate vaccine (PCV) should be administered first, followed by PPSV23 after an appropriate interval based on the patient's risk factors 6, 7, 8
- The optimal order and timing of pneumococcal vaccines depends on the patient's age 7, and presence of immunocompromising conditions or other risk factors 9
Standard Intervals Between PCV and PPSV23
- For most adults (non-immunocompromised): PCV followed by PPSV23 with ≥1 year interval 6, 7
- For immunocompromised adults: PCV followed by PPSV23 with ≥8 weeks interval 8, 9
Specific Recommendations Based on Patient Population
- For adults ≥65 years without previous pneumococcal vaccination, the first dose should be PCV21, PCV20, or PCV15, and if PCV15 is given, follow with PPSV23 ≥1 year later 6, 7
- For adults 19-64 years with immunocompromising conditions, the first dose should be PCV21, PCV20, or PCV15, and if PCV15 is given, follow with PPSV23 ≥8 weeks later 6, 8
- For adults 19-64 years with chronic medical conditions, the first dose should be PCV21, PCV20, or PCV15, and if PCV15 is given, follow with PPSV23 ≥1 year later 6, 7
Special Considerations for Patients with Prior Pneumococcal Vaccination
- If previously received PPSV23 only, wait ≥1 year after PPSV23 before administering PCV21, PCV20, or PCV15 6, 7
- If previously received PCV13 only, for non-immunocompromised adults, administer PPSV23 ≥1 year after PCV13, and for immunocompromised adults, administer PPSV23 ≥8 weeks after PCV13 6, 7, 8
Common Pitfalls to Avoid
- Overlooking risk factors: Immunocompromised patients need shorter intervals (≥8 weeks vs ≥1 year) between PCV and PPSV23 6, 8
- Unnecessary revaccination: If PCV21 or PCV20 is used, additional pneumococcal vaccination is generally not needed 6, 7
Evidence Quality and Considerations
- The recommendations are based on the most recent ACIP guidelines (2024), which represent the highest quality evidence available 6, 7, 8
- The newer conjugate vaccines (PCV15, PCV20, PCV21) have simplified pneumococcal vaccination schedules by reducing the need for sequential vaccination in many cases 6, 7
Pneumococcal Vaccination Interval in Elderly RA Patients
Key Recommendations
- The Centers for Disease Control and Prevention recommends a minimum interval of ≥8 weeks between PCV13 and PPSV23 administration for adults with immunocompromising conditions, including rheumatoid arthritis patients on immunosuppressive therapy 10
- For immunocompetent adults without immunocompromising conditions, the recommended interval is ≥1 year 10, 11
Immunocompromising Conditions
- The American College of Rheumatology considers rheumatoid arthritis patients on disease-modifying antirheumatic drugs (DMARDs) or biologic therapy as immunocompromised 12
- The 2023 ACIP guidelines explicitly include patients on immunosuppressive medications under immunocompromising conditions for pneumococcal vaccination purposes 10
Vaccination Timing
- Vaccination should ideally occur before initiating biologic therapy when possible, though killed/inactivated vaccines like PCV13 and PPSV23 can be given during treatment 12
- If the patient is ≥65 years old, both vaccines are indicated regardless of RA status, but the shorter 8-week interval applies due to the immunocompromising condition 10, 11
