Ceftriaxone Dosing and Administration Guidelines

Standard Dosing Regimens

  • For adults with normal renal function, ceftriaxone 2g should be administered intravenously every 12 hours (total 4g daily) for serious infections, according to the Journal of Infection and Praxis Medical Insights guidelines 1, 2
  • The International Ascites Club recommends 2g every 6-8 hours IV for spontaneous bacterial peritonitis 3
  • Ceftriaxone's long half-life allows for once-daily dosing in many clinical scenarios, making it suitable for outpatient treatment, as stated in Clinical Infectious Diseases 4

Special Clinical Scenarios

  • For penicillin-resistant pneumococci, the Journal of Infection and Praxis Medical Insights guidelines recommend adding vancomycin or rifampicin to the ceftriaxone regimen 1, 2
  • For elderly patients with suspected meningitis, the Journal of Infection and Praxis Medical Insights guidelines suggest adding ampicillin to cover Listeria monocytogenes, in addition to ceftriaxone 1, 2
  • For immunocompromised patients, the Journal of Infection guidelines recommend adding ampicillin to ceftriaxone for empiric coverage of Listeria 1

Duration of Therapy and Monitoring

  • Treatment durations may need extension if the patient is not responding adequately, according to Praxis Medical Insights 2
  • Adverse effects requiring treatment discontinuation occur in approximately 2.9-5.0% of courses, with rash being the most common, as reported in Clinical Infectious Diseases 4
