Diagnostic Approach to Mediastinal Masses and Knee Tumors in Young Patients
Introduction to Diagnostic Considerations
- The American Cancer Society recommends that patients with a mediastinal mass and a tumor in the knee be evaluated for Ewing's sarcoma, lymphoma, and germ cell tumors, with Ewing's sarcoma being the most likely diagnosis, followed by lymphoma, and then germ cell tumors 1
Tumor Characteristics
- Ewing's sarcoma is the second most common primary malignant bone tumor in children and adolescents, with a median age of diagnosis around 15 years, and can present with simultaneous mediastinal and bone involvement, according to the British Journal of Cancer 1
- The National Comprehensive Cancer Network states that Ewing's sarcoma predominantly affects males (1.5:1) and is less common in people of Chinese or African origin 1
- Although typically arising in bone, Ewing's sarcoma can present with extensive mediastinal metastases, as reported by the British Journal of Cancer 1
- The knee is a common location for osteosarcoma (the most frequent tumor around the knee), but Ewing's sarcoma can also affect this region, according to the British Journal of Cancer 1
- Sarcomas with CIC rearrangements and BCOR alterations (previously classified as Ewing type) have a poor response to chemotherapy and unfavorable prognosis, as stated by the British Journal of Cancer 1
Diagnostic Evaluation
- The National Comprehensive Cancer Network recommends that CT of the chest with contrast be the primary imaging modality for evaluating mediastinal masses, with the addition of MRI for tissue characterization, which can help prevent unnecessary biopsies 5
- PET-CT is optional but can help differentiate between benign and malignant disease, according to the National Comprehensive Cancer Network 5
- Imaging of the knee should include MRI to evaluate tumor extension and soft tissue involvement, as recommended by the British Journal of Cancer 1
Tissue Diagnosis
- The method for establishing a diagnosis should be the one with the most favorable risk-benefit ratio, as stated by the Chest journal 6
- Bronchoscopy with TBNA guided by EBUS has a diagnostic yield of 93% (95% CI, 91%-94%) and specificity of 100% for mediastinal masses, according to the Chest journal 6
- Transthoracic aspiration guided by CT of mediastinal masses can be performed safely, as reported by the Chest journal 6
- In patients with metastatic disease, FNA or core biopsy of the metastatic site (knee) represents the most efficient way to make a diagnosis and confirm staging, according to the Chest journal 6
Serum Markers
- Beta-HCG and AFP can be used to identify germ cell tumors, as recommended by the National Comprehensive Cancer Network 5
- These markers help differentiate between various anterior mediastinal tumors, according to the National Comprehensive Cancer Network 3
Prognostic Factors
- Ewing's sarcoma has metastatic rates of at least 50% in high-grade presentations, as reported by the British Journal of Cancer 1
- The presence of lymphadenopathy and direct mediastinal invasion are characteristics reported in approximately 78% of cases of aggressive mediastinal tumors, according to the Chest journal 4
