DMARDs and Abnormal Uterine Bleeding
Evidence from Reproductive Health Guidelines
- The European League Against Rheumatism (EULAR) and the American College of Rheumatology (ACR) guidelines do not identify menstrual irregularities or abnormal uterine bleeding as adverse effects of DMARDs requiring monitoring or management in women of reproductive age 1, 2, 3
- The ACR guidelines focus on teratogenicity, fertility effects, and pregnancy outcomes, but do not mention abnormal uterine bleeding as a potential adverse effect of DMARDs 1, 2
Key Safety Considerations for DMARDs in Women
- Methotrexate is associated with hepatotoxicity, bone marrow suppression, and teratogenicity, and requires folic acid supplementation and cessation 1-3 months before conception 1, 2
- Leflunomide is associated with hepatotoxicity, teratogenicity, and diarrhea, and requires cholestyramine washout due to its long half-life 1, 2
- Sulfasalazine is generally well-tolerated and compatible with pregnancy and lactation 1, 2
- Hydroxychloroquine has minimal toxicity and is compatible throughout pregnancy and lactation 1, 2
Monitoring Requirements
- Standard DMARD monitoring protocols focus on complete blood counts, liver transaminases, and serum creatinine at specific intervals, but do not include assessment for menstrual changes or uterine bleeding 2, 4
- Monitoring intervals are every 2-4 weeks for the first 3 months, every 8-12 weeks for 3-6 months, and every 12 weeks beyond 6 months 2, 4
Clinical Implications
- If a patient on DMARDs presents with abnormal uterine bleeding, the evaluation should proceed as it would for any woman with this complaint, as DMARDs themselves are not implicated 1, 2, 3, 4
- Underlying autoimmune disease activity, concurrent medications, and standard gynecologic causes should be considered as potential causes of abnormal uterine bleeding 3, 4
- Glucocorticoids, which are often used as adjunctive therapy with DMARDs, can cause menstrual irregularities and are a more likely culprit for menstrual changes than DMARDs themselves 4, 5
