Magnesium and Melatonin Supplementation for Pediatric Sleep

Introduction to Magnesium Dosing

  • The American Academy of Pediatrics recommends a standard supplementation dose of approximately 5-6 mg/kg/day of elemental magnesium for children, which translates to 110-165 mg daily for an average-weight 11-year-old child 1

Dosing and Administration

  • For an 11-year-old child, the starting dose of elemental magnesium is 110-165 mg daily, administered 1-2 hours before bedtime 1
  • The maximum safe dose should not exceed approximately 110-130 mg to minimize gastrointestinal effects in younger children, though an 11-year-old may tolerate slightly higher doses 1
  • Administer magnesium 1-2 hours before habitual bedtime 2

Melatonin as an Alternative

  • The American Academy of Sleep Medicine supports the use of melatonin for pediatric sleep disorders, with optimal dosing at 0.15 mg/kg (approximately 4-6 mg for an 11-year-old), taken 1.5-2.0 hours before bedtime 2
  • Melatonin doses of 3-6 mg have shown significant improvements in sleep latency, sleep duration, and sleep anxiety in children aged 6-12 years, with evidence quality rated as MODERATE to LOW but consistently positive 2, 3, 4

Magnesium Formulation and Safety

  • Magnesium bisglycinate is preferred for better gastrointestinal tolerance 1, 5
  • Avoid magnesium chloride in high doses due to increased anion gap and metabolic acidosis risk 6
  • Assess renal function before initiating magnesium supplementation, as it is contraindicated in severe renal impairment 1, 5
  • Monitor for gastrointestinal effects, such as diarrhea and abdominal cramping, which are primary dose-limiting factors 1, 5

Contraindications and Precautions

  • Do not use magnesium supplementation in children with severe renal impairment, complete heart block, or severe cardiac conduction abnormalities, or active gastrointestinal obstruction 1, 5

Clinical Recommendation Algorithm

  • Consider melatonin as first-line therapy for pediatric sleep disorders, due to its stronger evidence base 2
  • If choosing magnesium, start with 5 mg/kg/day elemental magnesium (approximately 130-150 mg for a typical 11-year-old) and use magnesium bisglycinate formulation, administered 1-2 hours before bedtime 1, 2
