Hypertension Management with Telmisartan/Hydrochlorothiazide

Initial Therapy and Dosing

  • The European Society of Cardiology recommends combination therapy as initial treatment for most patients with confirmed hypertension (BP ≥140/90 mmHg), with preferred combinations being a RAS blocker (ARB like telmisartan) with either a dihydropyridine calcium channel blocker or a thiazide/thiazide-like diuretic, with a target BP of 120-129 mmHg systolic 1, 2
  • The American Heart Association suggests that fixed-dose single-pill combinations, such as telmisartan/HCTZ, are recommended to improve adherence 1
  • For patients with BP >20/10 mmHg above target, initiating two-drug combination therapy (including telmisartan/HCTZ) is appropriate, with a goal of achieving target BP 3, 4

Special Populations

  • The American College of Cardiology recommends initial combination therapy with a thiazide diuretic and either an ARB or calcium channel blocker for black patients with hypertension, making telmisartan/HCTZ an appropriate first-line option 5, 6
  • For elderly patients (≥65 years), target systolic BP <130 mmHg if tolerated, though <140/80 mmHg is acceptable in those ≥85 years or with frailty, according to the European Society of Cardiology 1, 7
  • For patients with diabetes or metabolic syndrome, telmisartan/HCTZ is particularly appropriate as ARBs are associated with lower incidence of new-onset diabetes, and target BP <130/80 mmHg 7, 5, 8

Contraindications and Precautions

  • The American Heart Association advises to avoid telmisartan/HCTZ in patients with bilateral renal artery stenosis, and to avoid combination with ACE inhibitors (dual RAS blockade increases risks of hypotension, hyperkalemia, and renal dysfunction) 1, 5
