First-Line Nebulized Treatment for Pediatric Wheezing

Primary Treatment Algorithm

  • The American Thoracic Society recommends nebulized albuterol 5 mg (or 0.15 mg/kg) as the standard first-line treatment for acute wheezing episodes in children, administered every 20 minutes for up to 3 doses initially 1, 2
  • Alternative delivery: MDI with spacer (100 mcg per actuation, up to 20 actuations) is equally effective and preferred when tolerated, as it is cheaper and more convenient than nebulization 1
  • The American Academy of Allergy, Asthma, and Immunology recommends adding oral prednisolone 2 mg/kg/day (maximum 40 mg/day) for 3 days concurrently with bronchodilator therapy to establish control 1, 2
  • Corticosteroids are essential even in first-line treatment to prevent deterioration and reduce inflammation, with a strength of evidence rated as high 2

Severity-Based Treatment Escalation

  • Features requiring immediate nebulization include being too breathless to talk or feed, respirations >50/min, pulse >140/min, use of accessory muscles of breathing, and peak expiratory flow <50% predicted (if measurable) 1
  • Second-line adjunctive therapy includes adding nebulized ipratropium 250 mcg every 6 hours if inadequate response to initial albuterol doses, particularly in the emergency department setting 1, 2
  • Ipratropium provides additive benefit to beta-agonists in acute settings but should not be used as monotherapy, with a strength of evidence rated as moderate 2

Special Condition: Croup vs. Asthma/Wheeze

  • For croup specifically (not typical wheezing), oral dexamethasone 0.6 mg/kg is first-line, NOT albuterol, according to the American Academy of Pediatrics 5
  • Nebulized epinephrine 0.5 ml/kg of 1:1000 solution is used for severe croup to avoid intubation, but effect is short-lived (1-2 hours) 1
  • Nebulized budesonide 500 mcg may reduce croup symptoms within 2 hours, with a strength of evidence rated as low 1, 5
