Antibiotic Therapy for Spondylodiscitis
Diagnostic Approach Before Empiric Treatment
- The Infectious Diseases Society of America (IDSA) guidelines suggest withholding empiric antibiotic therapy in hemodynamically stable patients without neurological compromise until a microbiological diagnosis is established 2, 1
- In patients with a negative biopsy, a second image-guided biopsy with specific cultures for difficult-to-cultivate organisms, such as mycobacteria, Brucella, fungi, and anaerobes, should be performed 1, 4
- Consideration of a patient's risk factors, such as unstable living conditions, potential malnutrition, and exposure to temporary resources, is crucial in the diagnosis of tuberculosis 3
- Brucella infection should be considered in patients who consume unpasteurized dairy products 3
- Fungal infections should be considered in patients with immunosuppression, diabetes, or a history of intravenous drug use 2, 4
Initiation of Empiric Treatment
- Empiric antibiotic therapy should only be initiated in patients who develop hemodynamic instability, sepsis, progressive or severe neurological deficits, or signs of spinal cord compression 1, 5
- The recommended empiric antibiotic regimen includes vancomycin 15-20 mg/kg IV every 12 hours, plus one of the following options: cefepime 2 g IV every 8-12 hours, meropenem 1 g IV every 8 hours, ertapenem 1 g IV every 24 hours, or ceftriaxone 2 g IV every 24 hours plus metronidazole 500 mg IV every 8 hours 5, 6
Surveillance and Treatment Duration
- While awaiting diagnostic results, patients should be monitored daily for temperature, neurological status, and pain, and CRP and VS should be checked after 4 weeks as primary means of evaluating response 5, 7
- The initial parenteral therapy should be administered for 2-4 weeks, followed by oral antibiotics to complete a total of 6-12 weeks of treatment 5, 6
Surgical Considerations
- Surgery is not currently indicated in patients without neurological compromise, but future surgical indications to monitor include progressive neurological deficits with spinal cord compression, spinal deformity, instability, failure of conservative treatment, and unreliable pathogen identification requiring debridement for sampling 2, 4, 5, 7
