Treatment of Treatment-Resistant OCD with Comorbid ADHD
Introduction to Treatment Approach
- The American College of Cardiology is not relevant here, however, the treatment approach for patients with treatment-resistant OCD and comorbid ADHD involves treating the ADHD first with stimulants, then adding CBT with exposure and response prevention, and considering antipsychotic augmentation if these approaches fail 1
Step 1: Treat ADHD First with Stimulants
- Use methylphenidate or amphetamines as first-line therapy for ADHD, starting with extended-release formulations for around-the-clock coverage, and adjust dosing based on response 2
- Consider atomoxetine or alpha-2 agonists if stimulants are contraindicated or poorly tolerated, with guanfacine requiring 2-4 weeks until effects are observed and atomoxetine requiring 6-12 weeks 2
Step 2: Optimize OCD Treatment
- Confirm the patient has failed at least two SSRIs at maximum tolerated doses for 8-12 weeks each before declaring treatment resistance, with OCD requiring higher SSRI doses than depression or other anxiety disorders 1
- All SSRIs show similar efficacy, so choose based on side effect profile and drug interactions 3
- Adding CBT to pharmacotherapy shows larger effect sizes compared to antipsychotic augmentation, with 80% of patients receiving CBT augmentation responding versus only 23% with risperidone augmentation 1
- Deliver 10-20 sessions of individual or group CBT, in-person or via internet-based protocols, with patient adherence to between-session homework being the most robust predictor of good outcome 3, 4
Step 3: Antipsychotic Augmentation if Steps 1-2 Fail
- Risperidone and aripiprazole have the strongest evidence for SSRI-resistant OCD, with approximately one-third of patients showing clinically meaningful response to antipsychotic augmentation 1, 4
- Monitor metabolic parameters including weight, glucose, and lipid profiles at every visit, and allow adequate trial duration, with full therapeutic effect may be delayed until 5 weeks or longer 1, 3, 4
Step 4: Consider Clomipramine Switch
- If multiple SSRIs have failed, switch to clomipramine, the most efficacious medication for OCD, but use with extreme caution given cardiac risks, drug interactions, and tolerability concerns 1, 4
- Maintain treatment for 12-24 months after achieving remission due to high relapse rates 1, 3
Step 5: Neuromodulation for Highly Resistant Cases
- Deep repetitive transcranial magnetic stimulation (rTMS) is FDA-approved for treatment-resistant OCD with moderate effect size and 3-fold increased likelihood of response versus sham 1, 3
- Consider rTMS when multiple medication trials and CBT have failed, and refer to intensive outpatient or residential OCD treatment programs when standard approaches fail 1, 4
