Guidelines for Referral and Management of Renal Cysts

1. Indications for Interventional Radiology Referral

  • Percutaneous catheter drainage (PCD) and ethanol‑based sclerotherapy performed by interventional radiologists are the first‑line minimally invasive treatments for symptomatic simple renal cysts. 1
  • Patients who are poor surgical candidates should be referred to interventional radiology for minimally invasive percutaneous options. 2

2. Indications for Urology Referral

  • Complex cystic renal masses classified as Bosniak III or IV require urologic evaluation for potential malignancy and possible definitive surgical management. 3
  • When percutaneous drainage fails, surgical drainage (open or laparoscopic) should be undertaken by a urologist. 1
  • Renal or perirenal abscesses that may need surgical drainage or nephrectomy are best managed by urology. 2
  • Evaluation of infected or complicated cysts for possible nephrectomy should be performed by a urologist. 1

3. Outcomes and Recurrence

  • Surgical drainage becomes necessary when PCD cure rates are inadequate, reported at 14–32 % for complex cysts, or when definitive surgical management is indicated. 2
  • Surgical approaches have lower recurrence rates (2.5–5 %) compared with percutaneous techniques, but they are associated with higher morbidity. 1
