Treatment of Depersonalization-Derealization Disorder (DPDR) with Panic

Introduction to DPDR Treatment

  • The American College of Psychiatrists recommends initiating treatment with cognitive-behavioral therapy (CBT) focused on catastrophic misinterpretations of dissociative symptoms, combined with an SSRI (escitalopram, paroxetine, or sertraline) to address the underlying panic disorder, with a moderate level of evidence supporting this approach 1, 2

Treatment Algorithm

  • The treatment algorithm for DPDR that began with panic attacks involves addressing the panic disorder as the primary target, with the goal of reducing dissociative symptoms, and is supported by evidence from the MMWR Recommendations and Reports 1, 3
  • Start an SSRI immediately, with escitalopram, paroxetine, or sertraline as first-line options for panic disorder, based on recommendations from the PLoS Medicine journal 1, 2
  • Concurrent CBT referral is recommended, with a therapist skilled in both panic disorder and dissociation, structured as approximately 14 sessions over 4 months (60-90 minutes each), as suggested by Praxis Medical Insights: Practical Summaries of Clinical Guidelines 2
  • Avoid benzodiazepines for initial treatment, as they carry risks of dependence and do not address DPDR, according to the MMWR Recommendations and Reports 1, 3

CBT Framework for DPDR with Panic

  • CBT should target catastrophic misinterpretations of depersonalization/derealization symptoms, safety behaviors, and sensory grounding techniques to prevent dissociation during panic episodes, with a high level of evidence supporting the use of sensory grounding techniques 4, 5, 6, 7
  • Anxiety management strategies, including breathing techniques, progressive muscle relaxation, grounding strategies, visualization, distraction, thought reframing, and mindfulness, are also recommended, based on evidence from the Journal of Neurology, Neurosurgery and Psychiatry 4, 5, 6, 7

Medication Considerations

  • SSRIs are the cornerstone of pharmacotherapy for DPDR, though evidence specifically for DPDR is limited, and are recommended by the PLoS Medicine journal 1
  • If initial treatment fails, consider switching SSRIs or adding lamotrigine, based on recommendations from Praxis Medical Insights: Practical Summaries of Clinical Guidelines 2

Critical Clinical Considerations

  • Common pitfalls to avoid include treating DPDR in isolation, overreliance on benzodiazepines, underutilizing CBT, and premature discontinuation of antidepressant treatment, as warned by the PLoS Medicine journal and MMWR Recommendations and Reports 1, 3
  • Monitor for comorbid mood disorders, substance use, and childhood trauma, and address these issues accordingly, with a moderate level of evidence supporting the importance of monitoring for comorbidities 5
