TSH Stimulation for Radioactive Iodine Therapy

Mechanism and Target TSH Level

  • High TSH levels (≥30 mIU/L) are required before radioactive iodine therapy because TSH directly stimulates iodine uptake by thyroid follicular cells, maximizing RAI accumulation in target tissue and optimizing therapeutic efficacy, as recommended by the American College of Radiology, the National Comprehensive Cancer Network, and the American Thyroid Association 1, 2
  • The American Thyroid Association recommends a target TSH level of ≥30 mIU/L at the time of RAI administration, with a standard protocol of 0.9 mg IM on Day 1 and Day 2, followed by RAI on Day 3, using recombinant human TSH (rhTSH/Thyrogen) 1, 2
  • TSH stimulation is essential for remnant ablation after total thyroidectomy, treatment of known metastatic disease, diagnostic whole-body scanning, and stimulated thyroglobulin testing for surveillance, with a recommended TSH level of ≥30 mIU/L 1, 4

Clinical Context and Risk-Stratified RAI Dosing

  • The National Comprehensive Cancer Network recommends risk-stratified RAI dosing with TSH stimulation, with high-risk patients receiving 100-200 mCi, intermediate-risk patients receiving ≥100 mCi, and low-risk patients receiving 30-100 mCi 1, 4
  • The American College of Radiology recommends that TSH stimulation is critical for remnant ablation, treatment of known metastatic disease, and diagnostic whole-body scanning, with a recommended TSH level of ≥30 mIU/L 1

Methods to Achieve TSH Elevation

  • The American Thyroid Association recommends two approaches to achieve adequate TSH stimulation: thyroid hormone withdrawal (THW) and recombinant human TSH (rhTSH/Thyrogen), with rhTSH being the preferred method due to superior patient tolerance and quality of life 1, 2
  • The standard protocol for rhTSH is 0.9 mg IM on Day 1 and Day 2, followed by RAI on Day 3, which achieves equivalent TSH stimulation while maintaining euthyroid state 1
