Management of Nesidioblastosis

Medical Management

  • GLP-1 receptor antagonists, such as Exendin 9-39, have shown promise in correcting hypoglycemia in similar conditions, consistent with the role of GLP-1 in postprandial hypoglycemia, although evidence is still limited and based on small studies 1
  • Continuous glucose monitoring (CGM) is an important tool for individuals at risk of severe hypoglycemia due to nesidioblastosis, as recommended by clinical guidelines for managing hypoglycemia 2
  • Conservative management approaches, including medical therapy, should be pursued before attempting surgical intervention, according to clinical guidelines for managing nesidioblastosis 3

Special Considerations

  • Nesidioblastosis has been associated with post-Roux-en-Y gastric bypass hypoglycemia, although this connection remains controversial, and some studies suggest that the development of hyperinsulinemic hypoglycemia after gastric bypass surgery is not accompanied by islet hyperplasia or increased beta-cell turnover 1
