Management of Hypertension in CKD Stage 3 with Diabetes

Primary Recommendation: Combination Therapy

  • The American College of Cardiology and American Heart Association recommend a blood pressure target of <130/80 mm Hg in CKD patients, which often requires combination therapy, with losartan 50-100 mg plus HCTZ 12.5 mg daily 1, 2

If True Switching is Required: Choose Losartan

  • The American Heart Association suggests that losartan provides essential renoprotection that HCTZ cannot offer, reducing progression to end-stage renal disease and doubling of serum creatinine in type 2 diabetic patients with proteinuria 2

HCTZ Limitations in CKD Stage 3

  • The American Journal of Kidney Diseases states that thiazide diuretics are often perceived as ineffective in advanced CKD, though chlorthalidone may retain some efficacy at eGFR <30 mL/min/1.73 m², and HCTZ lacks the specific renoprotective benefits of RAS inhibition 2, 5

Dosing and Titration Strategy

  • The KDIGO guidelines recommend titrating ACE inhibitors or ARBs to the highest approved dose tolerated, with losartan starting at 50 mg daily and titrating to 100 mg daily (maximum approved dose) based on tolerance, and monitoring serum creatinine and potassium within 2-4 weeks of initiation or dose increase 2, 6

Critical Monitoring Parameters

  • The Annals of Internal Medicine suggests checking serum creatinine and potassium 2-4 weeks after starting losartan or changing dose, and continuing losartan if serum creatinine increases <30% from baseline, potassium remains <5.5 mEq/L, and no symptomatic hypotension occurs 2

Important Contraindications and Pitfalls

  • The Annals of Internal Medicine warns that dual RAS blockade with losartan and an ACE inhibitor increases risks of hyperkalemia, hypotension, and acute kidney injury without additional benefit, and should be avoided 2

Managing Hyperkalemia if it Develops

  • The Annals of Internal Medicine recommends implementing measures such as moderate dietary potassium intake, initiating or increasing loop diuretic dose, adding sodium bicarbonate if metabolic acidosis is present, and considering gastrointestinal cation exchangers if hyperkalemia occurs 2

Blood Pressure Target

  • The American College of Cardiology and American Heart Association recommend a target blood pressure of <130/80 mm Hg in patients with CKD stage 3, supported by Class I, Level B-R evidence for systolic BP, and reinforced by KDOQI commentary 1, 5
