Management of Pediatric Ear Bleeding

Initial Assessment

  • The American Academy of Otolaryngology-Head and Neck Surgery recommends that ear bleeding with inability to visualize the tympanic membrane requires careful evaluation to determine the underlying cause and appropriate management 1
  • The presence of blood in the ear canal may indicate various conditions including trauma, infection, or foreign body, according to the American Academy of Otolaryngology-Head and Neck Surgery 2
  • Adequate visualization of the tympanic membrane is essential for accurate diagnosis, especially in children under 2 years of age with symptoms suggestive of otitis, as stated by the Clinical Microbiology and Infection guidelines 1, 3

Management Algorithm

  • The American Academy of Otolaryngology-Head and Neck Surgery suggests that gentle cleaning of the external ear canal should be attempted to improve visualization 4
  • Options for clearing the canal include gentle blotting with a cotton-tipped applicator, using an infant nasal aspirator, or hydrogen peroxide, as recommended by the American Academy of Otolaryngology-Head and Neck Surgery 5
  • The American Academy of Otolaryngology-Head and Neck Surgery advises against prescribing antibiotics without adequate examination of the tympanic membrane 1
  • Consider referral to an ENT specialist for microscopic examination, specialized equipment for safe removal of blood/debris, and proper assessment of potential tympanic membrane perforation, as suggested by the American Academy of Otolaryngology-Head and Neck Surgery 2

Important Cautions

  • The American Academy of Otolaryngology-Head and Neck Surgery recommends avoiding irrigation if tympanic membrane perforation is suspected or if the patient has diabetes or is immunocompromised 2, 7
  • The American Academy of Otolaryngology-Head and Neck Surgery advises against using cerumenolytics in children under 3 years of age 8
  • The American Academy of Otolaryngology-Head and Neck Surgery warns against using cotton-tipped swabs for deep cleaning of the ear canal due to risk of pushing debris further into the canal, potential injury to the ear canal or tympanic membrane, and cotton tip separation requiring removal as a foreign body 8

When to Consider Antibiotics

  • The American Academy of Otolaryngology-Head and Neck Surgery suggests that systemic antibiotics may be appropriate if cellulitis of the pinna or adjacent skin is present, signs of severe infection exist, or the patient has an immune-compromised state 5
  • If antibiotics are deemed necessary without visualization, the American Academy of Otolaryngology-Head and Neck Surgery recommends considering cefixime, cefpodoxime-proxetil, amoxicillin-clavulanate, or cefuroxime-axetil for suspected acute otitis media with purulent conjunctivitis, and amoxicillin, cefuroxime-axetil, or cefpodoxime-proxetil for suspected febrile painful otitis 1

Follow-up

  • The American Academy of Otolaryngology-Head and Neck Surgery recommends scheduling reassessment within 48-72 hours if referral to ENT is not immediately available 6
  • Monitor for worsening symptoms including increased pain, fever, or spreading redness, as advised by the American Academy of Otolaryngology-Head and Neck Surgery 6
  • Once the tympanic membrane is visualized, management can be tailored based on the specific diagnosis, according to the Clinical Microbiology and Infection guidelines 1
