Acetaminophen for Renal Protection in Cardiac Surgery
Clinical Application and Dosing
- The American Society of Anesthesiologists recommends acetaminophen as a cornerstone of multimodal analgesia after cardiac surgery, with evidence supporting its additional benefit for kidney protection 2, 3
- Standard dosing of acetaminophen for adults after cardiac surgery is 1g every 6 hours, with intravenous administration preferred until gut function has recovered 1, 2
Advantages Over Other Analgesics
- Acetaminophen has fewer adverse effects on renal function compared to NSAIDs, which are associated with renal dysfunction after cardiac surgery 2, 3
- Acetaminophen provides opioid-sparing effects, reducing the risk of opioid-related adverse effects such as respiratory depression, nausea, vomiting, and ileus 2, 1
Monitoring and Precautions
- The American College of Cardiology and other guideline societies are not mentioned, however, standard monitoring of liver function is recommended, particularly in patients with pre-existing hepatic disease 4
- Acetaminophen should be used as part of a multimodal approach to pain management rather than as monotherapy 1, 2
