Propranolol Dosage and Administration

Introduction to Propranolol Therapy

  • The British Journal of Dermatology recommends starting propranolol at 1 mg/kg/day and increasing to a maintenance dose of 2-3 mg/kg/day with a minimum interval of 24 hours between dose increases 1, 2
  • The starting dose should be divided into three doses, and a 5 mg/5 mL preparation should be used to minimize dosing errors 1

Dosing Considerations

  • The minimum time between dose increases should be 24 hours 1
  • The target maintenance dose is 2 mg/kg/day for uncomplicated patients, and the maximum dose for non-responders is 3 mg/kg/day 1, 2
  • A more cautious starting and incremental dosing schedule should be used, with consideration of starting at 0.5 mg/kg/day 1
  • Individual dosing regimens should be determined by the supervising physician 1

Special Considerations

  • For patients at risk of hypoglycemia, blood glucose checks should be performed, and propranolol should be administered with or after feeding to reduce hypoglycemia risk 1, 2
  • Doses should be held during times of diminished oral intake or vomiting 2
  • Propranolol can be divided into two or three daily doses at the clinician's discretion 1

Monitoring and Titration

  • Normal heart rate ranges should be considered when monitoring response, with ranges of 72-104 beats/min for infants and 86-106 beats/min for toddlers 1
  • Normal blood pressure ranges should guide titration decisions, with ranges of 72-104/37-56 mmHg for infants and 86-106/42-63 mmHg for toddlers 1
  • Monitor for bradycardia during titration, especially in patients with reduced intravascular volume 3

Treatment Duration and Discontinuation

  • In infantile hemangioma cases, treatment can typically be stopped at 1 year of age 1
  • It is safe to stop propranolol abruptly rather than weaning gradually 1
