Cardiac MRI in Ischemic Cardiomyopathy: Comprehensive Assessment

Core Diagnostic Capabilities

  • The American College of Radiology recommends cardiac MRI as the gold standard imaging modality for ischemic cardiomyopathy, providing comprehensive tissue characterization, precise quantification of myocardial scar burden, assessment of viability for revascularization decisions, and superior prognostic stratification—all in a single examination 1
  • Late gadolinium enhancement (LGE) imaging identifies infarcted myocardium with unparalleled accuracy, detecting even small subendocardial infarctions as small as <2% of left ventricular mass, according to the American College of Cardiology 2
  • The extent of LGE independently predicts risk of malignant arrhythmias and heart failure progression regardless of ejection fraction, as stated by the American College of Radiology 1, 4

Functional and Volumetric Assessment

  • Cardiac MRI provides the most accurate and reproducible measurements of left ventricular ejection fraction, volumes, and mass—superior to echocardiography, according to the American College of Radiology 5, 1

Acute vs. Chronic Ischemic Cardiomyopathy Assessment

  • Stress perfusion imaging (vasodilator or dobutamine) detects inducible ischemia in viable but jeopardized myocardium, identifying candidates for revascularization, as recommended by the American College of Radiology 1, 4

Advanced Tissue Characterization

  • T1 mapping (native and post-contrast) quantifies diffuse myocardial fibrosis and calculates extracellular volume (ECV), which increases in ischemic cardiomyopathy, according to the American College of Radiology 1, 4
  • These mapping techniques are more sensitive and reproducible than LGE for tracking changes with therapy, as stated by the American College of Radiology 1, 4

Prognostic Value

  • The extent of LGE is the strongest independent predictor of cardiac death in ischemic cardiomyopathy, outperforming ejection fraction alone, according to the American College of Radiology 1
  • LGE serves as substrate mapping for arrhythmogenic foci, predicting sudden cardiac death risk, as reported by the American College of Radiology 7, 8

Clinical Decision-Making Algorithm

  • Initial assessment: Cardiac MRI with contrast provides morphology, function, and tissue characterization in one examination, as recommended by the American College of Radiology 1, 3

Important Caveats

  • Cardiac MRI can now be safely performed in most patients with pacemakers/ICDs, though technical adjustments and specialized sequences (wide-band inversion recovery) are required to mitigate artifacts, according to the American College of Radiology 5, 4
  • Gadolinium contrast is contraindicated in severe renal dysfunction (eGFR <30), but native T1 mapping provides alternative tissue characterization, as stated by the American College of Radiology 1, 4
