Treatment for Bipolar 2 Disorder with Seasonal Affective Features During Depression

Pharmacological Treatment Algorithm

  • The American Academy of Child and Adolescent Psychiatry recommends beginning with a mood stabilizer, such as lithium or valproate, as the foundation of treatment for patients with bipolar 2 disorder and seasonal affective features who are currently depressed, with the addition of bright light therapy and possibly an SSRI antidepressant for moderate to severe depression 1, 2
  • For moderate to severe depressive episodes, the American Academy of Child and Adolescent Psychiatry suggests considering adding an SSRI antidepressant, such as fluoxetine, in combination with a mood stabilizer to prevent triggering mania or rapid cycling 1, 2

Duration of Treatment

  • The American Academy of Child and Adolescent Psychiatry recommends that maintenance treatment with mood stabilizers should continue for at least 2 years after the last bipolar episode 1, 2

Monitoring and Adjustments

  • Regular monitoring for treatment response, defined as a 50% reduction in depression severity, is essential, according to the American Academy of Child and Adolescent Psychiatry 3
  • If inadequate response occurs within 6-8 weeks, the American Academy of Child and Adolescent Psychiatry suggests considering treatment modification 3

Psychosocial Interventions

  • The American Academy of Child and Adolescent Psychiatry recommends that psychoeducation should be routinely offered to individuals with bipolar disorder and their family members/caregivers 1, 2

Important Considerations and Pitfalls

  • The American Academy of Child and Adolescent Psychiatry advises that antidepressants should never be used as monotherapy in bipolar disorder, as they may trigger manic episodes or rapid cycling, and SSRIs are preferred over tricyclic antidepressants when an antidepressant is needed 1, 2, 4
  • The Journal of the American Academy of Child and Adolescent Psychiatry suggests being guided by a patient's history of treatment response to a particular medication when selecting treatment, and avoiding unnecessary polypharmacy while recognizing that multiple agents are often required for optimal management 4, 5
