Septic Arthritis Management

Diagnostic Criteria

  • The absence of crystals in synovial fluid effectively rules out gout and pseudogout as primary diagnoses, according to Praxis Medical Insights 1

Immediate Management Steps

  • The American College of Physicians and the Infectious Diseases Society of America recommend obtaining blood cultures before initiating antibiotics in patients with suspected septic arthritis, as bloodstream infection may be present in acute septic arthritis 2
  • Broad-spectrum antibiotic coverage targeting common pathogens including S. aureus (including MRSA in high-risk populations) and gram-negative organisms should be initiated immediately after cultures are obtained, as recommended by the Infectious Diseases Society of America 2
  • Urgent orthopedic consultation for potential surgical intervention is necessary, as surgical intervention (arthroscopic or open drainage) is typically required for septic arthritis to prevent cartilage destruction and preserve joint function 2 3

Additional Diagnostic Workup

  • ESR and CRP should be ordered as baseline inflammatory markers, with a combination providing 93% sensitivity and 100% specificity when at least 2 of 3 markers (ESR, CRP, fibrinogen) are abnormal, according to Praxis Medical Insights 2 4 5
  • CRP has a sensitivity of 73-91% and specificity of 81-86% for joint infection 5
  • Complete blood count with differential should be ordered, although peripheral WBC may be normal in joint infections 4 5
  • Basic metabolic panel should be ordered to assess renal function before antibiotic dosing 1

Imaging

  • Plain radiographs of the affected joint should be obtained to establish a baseline and may reveal alternative diagnoses 2 3 4

Monitoring Response to Treatment

  • Serial CRP measurements are the most sensitive marker for monitoring infection response 5
  • Repeat joint aspiration should be considered if clinical improvement does not occur within 48-72 hours or if inflammatory markers plateau or increase after initial improvement 5
