Management of Penile Fibrosis from Intracavernosal Alprostadil

Introduction to Treatment Approaches

  • The European Association of Urology guidelines recommend low-intensity shockwave therapy (LI-SWT) for mild vasculogenic erectile dysfunction as a weak recommendation, but do not mention its use for treating established penile fibrosis from any cause 2
  • LI-SWT may be combined with PDE5 inhibitors or vacuum devices for enhanced erectile dysfunction treatment results, according to the European Association of Urology guidelines 2
  • The American College of Physicians and other guideline societies implicitly support watchful waiting as the primary approach for penile fibrosis caused by intracavernosal alprostadil injections, since over half of cases improve spontaneously without intervention 1
  • Patients with penile fibrosis from intracavernosal alprostadil injections should continue regular follow-up with penile examination at 3-month intervals, monitoring for penile nodules, plaques, curvature, and induration at each visit, as recommended by medical guidelines 1
  • Retraining proper injection technique is recommended to prevent further trauma in patients with penile fibrosis from intracavernosal alprostadil injections 1
  • Transition to PDE5 inhibitors, such as sildenafil, tadalafil, or vardenafil, can be considered if fibrosis progresses or erectile function deteriorates, as suggested by medical guidelines 1
  • Intraurethral alprostadil may be considered to avoid further corporal trauma in patients with penile fibrosis from intracavernosal alprostadil injections 1
  • Vacuum erection devices with vacuum limiters can be used to avoid additional penile trauma in patients with penile fibrosis from intracavernosal alprostadil injections 1
  • Penile prosthesis implantation may be considered only if erectile dysfunction becomes refractory to medical management and fibrosis significantly impairs quality of life, as recommended by medical guidelines 1

Epidemiology

  • The incidence of penile fibrosis with intracavernosal alprostadil ranges from less than 1% to 23.3% depending on the study, with more rigorous examination protocols detecting higher rates 3
