Atomoxetine and Clonidine Combination for ADHD

Introduction to Combination Therapy

  • The American Academy of Pediatrics recommends that both extended-release clonidine and atomoxetine can be safely combined with each other or with stimulants for ADHD treatment, with FDA approval specifically supporting clonidine as adjunctive therapy to stimulants, while atomoxetine has limited but supportive evidence for combination use 1

When to Use This Combination

  • Add clonidine to atomoxetine when atomoxetine monotherapy provides inadequate symptom control, particularly when ADHD symptoms persist despite optimized atomoxetine dosing after 6-12 weeks, as recommended by the American Academy of Pediatrics 2, 1
  • The combination is beneficial for patients with comorbid sleep disturbances, as clonidine's sedating properties become advantageous, according to the American Academy of Sleep Medicine 2, 3
  • The combination is also beneficial for patients with comorbid tics or Tourette syndrome, as both medications can help without exacerbating tics, as recommended by the American Academy of Pediatrics 2, 1

Specific Clinical Scenarios Favoring This Combination

  • The American Academy of Pediatrics recommends this combination for patients at risk of substance abuse, as both medications are non-controlled, eliminating diversion concerns 2, 3
  • The combination is beneficial for patients with comorbid oppositional defiant disorder or conduct disorder, as both medications show positive effects on disruptive behaviors, according to the American Academy of Child and Adolescent Psychiatry 2
  • The American Academy of Pediatrics recommends this combination for stimulant non-responders or those with stimulant contraindications, as it provides two complementary non-stimulant mechanisms 2, 1

Dosing Algorithm

  • The American Academy of Pediatrics recommends starting atomoxetine at 0.5 mg/kg/day for 3 days, then increasing to 0.8 mg/kg/day, with a target dose of 1.2 mg/kg/day (maximum 1.8 mg/kg/day or 100 mg/day, whichever is less) 1
  • The American Academy of Pediatrics recommends starting clonidine at 0.05-0.1 mg at bedtime, with titration by 0.1 mg weekly based on response and tolerability, and a maximum dose of 0.4 mg/day divided BID-TID 4, 3

Critical Safety Monitoring

  • The American Academy of Pediatrics recommends obtaining baseline blood pressure and heart rate before initiating either medication, and monitoring vital signs at each dose adjustment, as the opposing cardiovascular effects may partially offset each other but require ongoing surveillance 1, 3
  • The American Academy of Pediatrics recommends monitoring for jaundice, dark urine, right upper quadrant tenderness, or unexplained flu-like symptoms indicating potential hepatotoxicity (extremely rare) 1

Common Adverse Effects to Anticipate

  • The American Academy of Pediatrics notes that somnolence/sedation is the most common adverse effect of clonidine, occurring in the majority of patients initially, and that somnolence is additive when combining clonidine with atomoxetine 1, 3
  • The American Academy of Pediatrics recommends monitoring for emergence of hostility, agitation, or mood changes at each visit, as atomoxetine carries an FDA black box warning for suicidal ideation 1

Critical Safety Warning: Clonidine Discontinuation

  • The American Academy of Pediatrics warns that clonidine should never be abruptly discontinued, and recommends tapering by 0.1 mg every 3-7 days to avoid rebound hypertension, which can be severe and potentially life-threatening 1, 3

Evidence Supporting Combination Use

  • The American Academy of Pediatrics notes that while extended-release clonidine has FDA approval specifically for adjunctive use with stimulants, the evidence for combining clonidine with atomoxetine is more limited but supportive, with both medications recommended as second-line agents after stimulants 1, 2

Special Population Considerations

  • The American College of Obstetricians and Gynecologists recommends caution when using atomoxetine during breastfeeding, as there are no published breastfeeding studies, and limited data suggest no increased risk for major malformations with clonidine, but caution is advised during breastfeeding with infant monitoring for drowsiness and hypotonia 4, 3

Timeline Expectations

  • The American Academy of Pediatrics recommends counseling families that this is not a quick-fix approach, with atomoxetine requiring 6-12 weeks for full therapeutic effect, and clonidine requiring 2-4 weeks for noticeable benefits, and a total time to assess combination efficacy of 8-16 weeks minimum after reaching target doses of both medications 2, 3
