Droxidopa for Neurogenic Orthostatic Hypotension
Indications and Patient Selection
- The American College of Cardiology recommends droxidopa for treating symptomatic neurogenic orthostatic hypotension (nOH) in adults with Parkinson's disease, multiple system atrophy, pure autonomic failure, dopamine beta-hydroxylase deficiency, and non-diabetic autonomic neuropathy, though its effectiveness beyond 2 weeks remains unestablished and requires periodic reassessment 1
- Droxidopa is indicated for primary autonomic failure (Parkinson's disease, multiple system atrophy, pure autonomic failure) 2
Clinical Efficacy
- Droxidopa improves symptoms of nOH in the target populations and may reduce falls according to small studies 1, 3, 4
- The American Heart Association recommends considering droxidopa within a broader treatment algorithm for nOH, including first-line non-pharmacologic measures such as acute water ingestion, physical counter-pressure maneuvers, and compression garments 1, 3, 4
Drug Interactions and Special Populations
- Concurrent use of carbidopa may decrease droxidopa effectiveness in Parkinson's disease patients 1, 3, 4, 5, 6, 7, 2
Integration with Comprehensive nOH Management
- The American College of Cardiology/American Heart Association/Heart Rhythm Society guidelines recommend considering droxidopa as part of a comprehensive treatment plan for nOH, including non-pharmacologic measures and other pharmacologic options such as midodrine, fludrocortisone, and salt supplementation 1, 3, 4, 6, 2
