Diltiazem Discontinuation Guidelines

Patient Populations Requiring Caution

  • Patients with coronary artery disease, specifically those with unstable angina or non-ST elevation MI, may experience symptom recurrence if diltiazem is abruptly stopped, according to the European Heart Journal 1
  • Patients with coronary artery disease, such as those with unstable angina or non-ST elevation MI, may experience symptom recurrence if diltiazem is abruptly stopped, as reported in Circulation 2
  • Patients using diltiazem for rate control in atrial fibrillation should be cautious of rapid ventricular response if diltiazem is abruptly discontinued, as noted in guidelines from a medical insights society 3
  • Patients with heart failure, particularly those with severe LV dysfunction, should be monitored closely during diltiazem discontinuation, as recommended by Circulation 2 and another study in Circulation 4
  • The European Society of Cardiology implies that patients with stable conditions can have diltiazem stopped abruptly without tapering, but should be monitored for return of symptoms, based on evidence from the European Heart Journal and Circulation [1] [2]
  • High-risk patients, including those with unstable angina, recent non-ST elevation MI, poorly controlled atrial fibrillation, or variant angina, should consider gradual tapering of diltiazem, as suggested by guidelines from medical societies, with alternative medications ready if symptoms recur [2] [3]

Monitoring and Considerations

  • After discontinuation, patients should be followed for several days to monitor blood pressure and heart rate, and to be alert for return of original symptoms that diltiazem was treating, as recommended by medical guidelines 2
  • When transitioning to another rate-controlling agent, such as a beta-blocker, complete clearance of diltiazem should be ensured before starting the new medication to avoid synergistic effects on AV nodal conduction, as advised by a medical insights society 3
