Ciclopirox Treatment for Fungal Nail Infections

  • For fingernail infections, ciclopirox should be used as an 8% nail lacquer applied once daily for up to 24 weeks, and for toenail infections, up to 48 weeks of daily application is recommended 1, 2, 3

Efficacy and Outcomes

  • Ciclopirox lacquer once daily has shown mycological cure rates of 34% versus 10% with placebo, with clinical cure rates of 8% versus 1% with placebo 1, 2, 3
  • Cure rates are usually lower with ciclopirox compared to amorolfine, though no head-to-head trials exist 1, 2

Side Effects and Precautions

  • Periungual and nail fold erythema are the most common side effects of ciclopirox treatment 1, 2, 4

Special Populations

  • No specific dosing adjustments are recommended for elderly patients 5
  • Safety and efficacy in children have not been established 4

Potential Pitfalls

  • Clinical improvement may not correlate with mycological cure, which has invariably lower rates (often by 30%) 1, 2
  • Treatment failure may occur due to poor patient compliance, lack of drug penetration into the nail, or drug interactions 4
  • Ciclopirox is most appropriate for superficial and distal onychomycosis when systemic therapy is contraindicated 4, 5, 6
