Perioral Impetigo Treatment Guidelines
First-Line Treatment
- The Infectious Diseases Society of America recommends mupirocin 2% topical ointment for children with minor skin infections such as impetigo, applied three times daily for 5-7 days 1
- Mupirocin 2% ointment is the FDA-approved formulation for impetigo treatment, providing better adherence to moist perioral skin and maintaining longer contact time with the infected area compared to cream formulations 1
Treatment Protocol
- Apply mupirocin 2% ointment to the affected perioral area three times daily for 5-7 days, keeping lesions covered with clean, dry bandages when possible to prevent spread to others 2
- Re-evaluate after 48-72 hours if no improvement is observed 2
Alternative Treatments
- Retapamulin 1% ointment applied twice daily for 5 days is an effective alternative if mupirocin is unavailable or has failed 2
Oral Antibiotic Treatment
- Switch to oral antibiotics if the impetigo is extensive, topical therapy fails after 48-72 hours, or systemic symptoms develop, with options including cephalexin, clindamycin, or TMP-SMX 2
- Avoid tetracyclines in children under 8 years of age, although they may be technically permissible at 11 years of age 1
