Diagnostic Approach for Finger Pain

Initial Assessment

  • The American College of Radiology recommends obtaining a detailed pain history including onset, quality, intensity, distribution, duration, and exacerbating/relieving factors for patients with finger pain 4
  • The European League Against Rheumatism suggests assessing for systemic conditions that may present with finger pain, including rheumatoid arthritis, diabetes, and other inflammatory conditions 5

Diagnostic Imaging Algorithm

  • The American College of Radiology recommends radiographs as the first-line imaging examination for finger pain, which may be the only imaging needed to establish a diagnosis in many cases 1, 3, 6
  • For suspected soft tissue pathology, the American College of Radiology suggests MRI without IV contrast, which accurately depicts abnormalities of bones, bone marrow, articular cartilage, ligaments, synovium, and tendons 6, 7
  • For suspected inflammatory arthritis, the American College of Radiology recommends MRI with or without IV contrast, which helps identify active synovitis, tenosynovitis, and bone marrow edema 3, 6, 8
  • For suspected ganglion cyst or tendon abnormality, the American College of Radiology suggests ultrasound, which is particularly useful for examining extra-articular soft tissues and tendon abnormalities 6, 8

Treatment Approach

  • The American College of Rheumatology recommends acetaminophen, nonsteroidal anti-inflammatory drugs, physical activity, and exercise for patients with osteoarthritis 5, 9
  • The European League Against Rheumatism suggests pharmacological treatment according to rheumatology guidelines, physical activity, and exercise for patients with inflammatory arthritis 5
  • The American College of Rheumatology recommends considering orthotics (splints, braces) if pain impedes functioning for patients with osteoarthritis 5
