Management of Rheumatoid Arthritis with Pulmonary Involvement

Introduction to Treatment Guidelines

  • The American College of Rheumatology recommends avoiding methotrexate and leflunomide in patients with rheumatoid arthritis and active interstitial lung disease, instead prioritizing tocilizumab or rituximab as first-line biologic therapies, with close monitoring using high-resolution chest CT every 3-6 months 1

Pharmacological Strategy in Rheumatoid Arthritis with Pulmonary Involvement

  • The American College of Rheumatology suggests that abatacept can be considered as a third option in refractory cases 2

Treatment Algorithm

  • The European League Against Rheumatism recommends initiating tocilizumab 8 mg/kg IV every 4 weeks or 162 mg SC weekly, adding prednisone ≤7.5 mg/day for a maximum of 6 months, then rapidly reducing, and avoiding high doses of glucocorticoids (>10 mg/day) due to the risk of infections and osteoporosis 1, 2
  • The American College of Rheumatology suggests evaluating disease activity every 1-3 months using CDAI or SDAI until remission or low activity is achieved, with regular chest CT and pulmonary function tests every 6 months 3, 4

Non-Pharmacological Therapies

  • The American College of Rheumatology recommends supervised aerobic exercise and muscle strengthening to maintain functional capacity, as well as occupational therapy for joint protection techniques and assistive devices 5, 6
  • The American College of Rheumatology suggests that smoking cessation is obligatory, as smoking is a modifiable risk factor for rheumatoid arthritis and interstitial lung disease progression 7

Common Pitfalls to Avoid

  • The American College of Rheumatology advises against using methotrexate in patients with interstitial lung disease, as it is a relative contraindication 1
  • The European League Against Rheumatism recommends not delaying the initiation of biologics, as time is critical to prevent irreversible fibrosis in active interstitial lung disease 8
