Topical Tretinoin and Birth Defects

Key Evidence on Safety

  • The American Academy of Dermatology classifies topical tretinoin as Pregnancy Category C, stating "it is not known whether this drug is excreted in human milk" and noting safety has not been established in children under 10-12 years 1, 2

Clinical Management Recommendations

  • For pregnant women requiring acne treatment, safer alternatives include topical azelaic acid (Pregnancy Category B), topical erythromycin (Category B), topical clindamycin (Category B), and benzoyl peroxide, which have minimal expected systemic absorption and fetal harm 3, 4

Important Caveats

  • The distinction between topical and oral retinoids is critical: systemic isotretinoin is absolutely contraindicated in pregnancy (Category X) and requires mandatory pregnancy prevention programs 5
